I am trying to understant the process for a dep authorization to plug a well. By looking at this authorization is there anything else that needs to be filed before the well is plugged?

To plug a well in PA, the well operator needs to make DEP aware of where the well is. The way an operator does that is to apply for a "Permit to Plug and Abandon" from the DEP. That way, DEP knows where a plugged and abandoned well is, and the Commonwealth of PA makes some $$$ off of the process as well. And most importantly, the well is plugged properly as to not cross-contaminate any fresh water bearing zones.
