Give them the county, twp and well info and ask for the completion report and any other pertinent info while your at it. They email back fairly quickly. I was told last year that the completion reports aren't available right away after completion and that I should request again in a few months. Of course I never did do it.... maybe i'll send in some requests as well.

Permalink Reply by craig on January 18, 2013 at 3:48pm Another thing, by asking for any other information on a certain well you WILL get the actual finalized paperwork for the well permit once approved, that gives you various types of information.
Permalink Reply by Todd on January 22, 2013 at 7:54am They just sent me a completion report; however, there was no open flow data included, nor any specific shale thickness information... just generic drilling logs! Not very useful data whatsoever. They sent me a "Well Record".

Maybe they have changed some things?

Permalink Reply by craig on January 22, 2013 at 3:50pm That page above of the Burdick well was received before the well was completed. It had nothing to do with completion reports, which means maybe that type of paperwork is in a different data base.
I wonder if once they complete the well that the above paperwork isn't available anymore, dunno.
You might try and ask her if she can email you the well location plat and the vertical view from the PERMIT for the well. Look up the permit # so she has it. Or just ask for copies of the original permit.
